Think I have a trojan, can't get rid of it.

System is running Vista.
Symptoms: First was that some (not all) persons in my address book received a spam (or trojan?) email apparently from me. Had "Subject: Sep 11". Note that I don't use Outlook, I use Earthlink webmail.
Since then I have also noticed quite large amounts of outbound data on my internet connection, normally this would be close to zero.
I suspect the cause of all this is the fact that in a moment of gross misattention I accepted an Activex request on an incoming junk email.
I have followed all the steps in "Recommended Initial Steps - Cleaning Malware". The only product to find anything was Blacklight which found and quarantined one virus. But after that I still have data leaking out. It mostly seems to go out at the same time I enter or click on a new url in Explorer. Only a little data goes out when I first activate the internet connection, mostly it seems to flow once I start Explorer. I have noticed a few times that TrustedInstaller runs as soone as I start up Explorer, not normal surely?
