Yes, THGuard.exe runs in memory. It polls memory every 10 seconds searching for malicious files. If you do not use the "automatically remove trojan" option, you will get an alert box that THGuard.exe has found a specific trojan in memory. You then instruct THGuard to clean it or ignore it. This gives you the opportunity to examine anything that you wish...before you allow THGuard to Quarantine a malicous file.
